YOUR SOCIAL SECURITY

Request a copy of your Social Security Estimate & Earnings Statement from the Social Security Administration. You may obtain a copy by:

1.	Requesting a Social Security Statement online.
2.	Download a copy of the SSA-7004 request in pdf format. Once completed, mail to the Social Security Administration to the following:
	Social Security Administration Wilkes Barre Data Operations Center P.O. Box 7004 Wilkes Barre, PA 18767-7004 OR
3.	Contact the Social Security Administration at (800) 772-1213 and request your Social Security Estimate Statement.

*Note: It will take approximately 4 to 6 weeks to receive your statement.
